Please avoid sending detailed medical information through this form. Relevant suitability information can be discussed directly. Your inquiry is not published on the website or used to sign you up for marketing.

Spam prevention and website operation

The form uses a verification question, automated abuse checks, and temporary submission limits. Hashed technical identifiers and duplicate-message fingerprints are retained for up to 24 hours to limit abuse; message contents are not stored in the form’s website database. Messages forwarded by email are retained in our correspondence systems.

The hosting and security services used to operate this site may process standard technical information such as IP addresses and request logs. WordPress may use cookies for administrative sign-in. These pages do not add advertising trackers or embedded social-media feeds.
